It's easy to create nice looking forms with Forminator.

I can't believe the amount of features and customization you can do with Forminator without having to upgrade to a paid/pro plan. I've used it to build a few fairly complex forms, including repeaters and tons of dynamic fields, and haven't paid a penny. I might just have to donate so I can sleep at night.
